 Introducing Formedic's New and Exciting Charting Method! Formedic Medical History Questionnaire (MHQ) is software that gathers a patient's medical history and symptomatology, and presents this information to doctors in an easy-to-use, clinically organized format. MHQ outputs a customized, pre-filled patient record form on which a doctor can easily and quickly chart the entire patient visit.
The program uses its extensive medical knowledge-base to customize the questions it asks each patient. MHQ uses the patient's "chief complaint" or "reason for visit" to select an initial set of questions. As the patient goes through the questionnaire, the program analyzes the answer to each question and decides which follow-up questions to add to, or delete from, the questionnaire. MHQ then gathers and organizes the subjective medical information just as a doctor would.
MHQ is designed to be used in-office by patients every time they come in for an appointment, or via the Internet from the patient's home or office before their visit.
